Veterans Who Have Experienced Homelessness: Addressing their Social Determinants for Better Health

Saturday May 9, 2020, 1:00pm – 2:30pm

Homelessness is a powerful example of how life history and social circumstance influence health, and vice versa. Veteran homelessness declined by 50% from 2009 to 2016, partly because of US federal investments in housing, social and health care. But the health and social challenges faced by people who are homeless do not stop the moment they become housed. Come see how primary care and social work staff at Birmingham VA Medical Center address those challenges in a special post-meeting tour and presentation for SGIM meeting attendees and Dr. Carolyn Clancy, Deputy Under Secretary for Discovery, Education and Affiliate Networks. Meeting attendees are invited to sign up for a 90-minute presentation and tour by VA staff and patients, including a tour of the VA Homeless Patient-Aligned Care Team (H-PACT). Van transportation to and from the SGM meeting site (10-minute ride) will be provided.
