A 60-year-old Hispanic man with a history of insulin-dependent diabetes mellitus presented for six days of abdominal pain, nausea, and vomiting. Broadly, his symptoms can be categorized into two initial diagnostic categories: gastrointestinal etiologies or a gastrointestinal manifestation of systemic disease.
